Significant hyperkalemia may lead to life-threatening arrhythmias, thus requiring timely recognition and management. ECG is one of the immediately available tool to help identify hyperkalemia. Although, ECG changes may not correlate closely with serum potassium level.
ECG alterations that may be found in hyperkalemia include:
1. Tall, peaked T wave
2. Decrease or loss of p wave
3. Widening of the QRS complex with bizarre morphology
4. Double counting of heart rate by the ECG monitor
5. Wide-QRS PEA and Non-shockable pulseless wide complex tachycardia
Identifying acute ECG changes is necessary in monitoring critical care patients.
