The exercise stress test is a non-invasive modality to evaluate CAD. There is a number of options to perform the test, one of them is treadmill test. The selection of modality and protocols should be based on the patient’s functional capacity based on age, physical fitness, and underlying disease.
Some of the most common protocols used for treadmill test are the Standard Bruce protocol, Modified Bruce protocol, and Naughton protocol. Standard Bruce protocol is preferred for testing. Its outcomes are well validated, and exercise capacity measured in METs has good prognostic value. Modified Bruce protocol is for patients with predicted poor exercise capacity. And Naughton protocol allows for a more gradual increase in exertion used in older and deconditioned patients.
